*Please see the main office or athletic office to obtain a copy of the Activities Code of Conduct before beginning a sport.* 

Sports physicals are good for two years. So every other year the student will need the sports physical card, and then the alternate year card the following year.

Athlete Travel Release Form (allows a student to leave a sporting event with a parent instead of with the team)
